The Pupil
must be in possession of a valid full driving licence. |

Particulars
of the vehicle (type, cubic capacity, model, registration
number, distinguishing features) must be given to the school
below). |

The vehicle
should be comprehensively insured, in possession of a valid
MOT (if required), safe and complying with all Road Traffic
Act legal requirements. |
| 4. |
No responsibility
can be accepted for any loss or damage to the vehicle while
it is on the premises. |
| 5. |
The vehicle
may be used for journeys to and from home/college. It may
not be used for journeys within the boundary. Permission must
be obtained for occasional visits during the week. |
| 6. |
No lifts
may be given to other pupils without the Head's prior permission.
The Head will also need written permission from the parents/guardian
of the driver + passenger. |
| 7. |
Permission
may be withdrawn by the Head at any time, either for non-observance
of these regulations, or for any behaviour with the vehicle
which is considered unreasonable or detrimental to general
welfare. |
| 8. |
The
Head will require a photocopy of:
a) full driving licence
b) valid MOT
c) valid insurance details |

Permission to use the vehicle will not start until one copy
of this form has emailed to the Head and the form has been
signed and dated by the Headand returned to the parents. |
